GCCFSA Forms
If you are registering a litter, kitten or cat or transfering from one body to another, you will find the form you need here. Please ensure that you are using the CURRENT version of the form as PREVIOUS versions will no longer be accepted by the Registrars. You should also check the Price List for the fees applicable.
Most GCCFSA forms are available in PDF. Download the form, open with Acrobat Reader, complete the details and save the form - this file can now be printed and sent as an attachment to an email.

- R1 - Application for Registration of Litter (pdf) - Requires pedigree of each parent showing colour, pattern, breed and all registration numbers.
- R1 - Pedigree
- R2 - Application for Registration of Ownership (pdf) - *Breeding and Non-Breeding*
- R2D - Application for Registration of Group 5 OR Group 6 (docx) - Domestic or Companion registration
- R3 - Transfer of Ownership (pdf) - Transfer within GCCFSA, to be accompanied by original certificate
- R4 - Transfer of Registration (pdf) - Transfer from another council including FASA. Must be accompanied by original certified pedigree provided by previous registry (must be signed) showing cat plus at least four (4) generations of ancestry with registration number, colour, pattern and breed of each ancestor. Also to be accompanied by original certificate from previous registry showing registration in name of owner/applicant*.
- R5 - Application for Cattery Licence & Prefix (pdf) - Code of Ethics for Breeders and Owners on reverse
- R8 - Application for Honours Certificate (pdf) - Pedigree
- R8D - Application for Honours Certificate (pdf) - Domestic or Companion
- R9 - Application for Medallion Awards (pdf) - *Record of certificates on reverse must be completed*

ACF Title Application Forms
All forms to be submitted to the Secretary of GCCFSA with original certificates for verification. Fees are payable to the Australian Cat Federation.

- ACF National Title - Fee $25.00
- ACF Distinguished Merit Award - Free to apply
